The reality of managing diabetes takes place in daily life and not hospitals. It has been proven that Diabetes requires extensive care, medical follow-up and continued support. Currently, existing medical coverage does not provide initiatives that facilitate the advancement of self-management and support for people with diabetes.
A joint partnership aimed at supporting and improving diabetes self-management has been launched to help track progression, improve and assist families control this distressing and increasing phenomenon.
What is i-ACT?

MetLife Alico and Life Scan (part of Johnson & Johnson family of companies) are launching i-Act- a comprehensive Diabetes management program.
Through its simple steps, continuous medical support and interactive educational sessions, i-Act is a 12-month journey that can help people with diabetes comply with easy lifestyle shifts that will keep them healthy. Throughout their journey, people who enroll in the program will have access to:
- Diabetes Management Program
- Endocrinologists
- Laboratory test conveniently performed in the location of their choice
- Convenience of delivering the Medical Prescriptions to their doorsteps
- Interactive Educational Sessions

Who is Eligible to join this Program?
MetLife Alico cardholders and their dependents diagnosed with controlled or uncontrolled Diabetes can join this Program.
Enrollment is:
- Subject to terms and conditions of the policy
- Restricted to policies that cover pre-existing conditions
Why should people who are already seeing doctor for their diabetes consider joining the Program?
At present, patients with diabetes may be just getting treatment for one aspect of their disease.
i-Act is a comprehensive management program that will include access to continuous medical support while also equipping people with diabetes with the tools that can help them comply with simple lifestyle shifts to ensure a healthier future with their loved ones.
How to Enroll in i-Act?
MetLife Alico cardholders and their dependents diagnosed with Diabetes can enroll to the Program by simply:

- Choosing which facility / doctor they would like to work with. Click here to download the i-Act list of Medical Providers
- Fill-in the Participation Agreement Form. Click here to download the Participation Form
- Send the signed agreement form to ppoteam@metlifealico.com or fax it to 06 556-3308
Once the form is received, an SMS text will be sent to the applicant to confirm enrollment.
What happens after Enrollment?
After receiving confirmation of enrollment, insured members will need to:

- Schedule an appointment for lab tests (to be performed in the privacy of their home whenever it is convenient for them or at Medsol labs) by replying to the confirmation SMS.
- A lab specialist will perform lab tests at the time and location requested by the enrolled member
- Results of the lab tests will then be forwarded to the insured so that he can schedule an appointment for consultation
- Following the consultation with the doctor, the insured just needs to send his prescription to Aster Pharmacy either by email to metlifealico@asterpharmacy.com or by fax to 04-2672948 and they will arrange for delivery of medications to their doorstep. The i-Act kit which includes a Glucometer, strips, education materials and other tools will also be delivered to the insured along with the medication.
Does this Program come with any extra charges?
With i-Act, MetLife Alico is widening the scope of insurance coverage to include procedures and medications that are not usually covered under conventional insurance.
Enrolling in this comprehensive diabetes management program comes at no additional cost and does not change the terms of the policy coverage; therefore only the deductible and co-insurance are applicable as per the terms and conditions of the policy.
In summary, i-Act does not increase the insurance premium the employer is paying nor does it require the enrolled member to pay to benefit from the program’s scope.
i-Act is a 12-month Program, what happens when the cycles are completed?
i-Act is a journey to a healthier living. We have structured the program to last over a period of time to quantify reachable goals. People who have completed their yearly cycle are entitled to renew their enrollment as long as they remain covered by MetLife Alico.
Does the employer or any other party be informed of enrollment of the member?
No, medical records and enrollment details are confidential and privacy of this enrolled member is guaranteed. Only the physician who is monitoring the patient’s condition will have access to the insured’s medical records.
